Grand Rounds Forms

Instructions to Download and Save the Forms
Please click on the icon next to the version of the form for your platform (PC or Mac). PC Internet Explorer: click on the icon and the form will automatically open in your browser window. To save - select File>Save As... and save to your hard drive. Firefox: click on the icon and a dialogue box will appear. You should open with 'Word' and then 'Save As'. Mac Safari: clicking on the icon will launch a new window where the file will download. Click on the magnifying glass icon in this new window to launch the document. Firefox: click on the icon and a dialogue box will appear. You should open with 'Word' and then 'Save As'. Most forms are password protected and must be opened as 'Read Only' (if prompted) and saved using the 'Save As' function.
